Registration of Birth (Timely Registration)

The birth of a child shall be registered within 30 days from the time of birth at the Local Civil Registry office of the municipality where the birth occurred, beyond this period will be for delayed registration

Requirements

  • Duly filled-up Certificate of Live Birth
  • Certificate of Marriage

Steps

  1. Sign in the client log book in the office
  2. Bring the filled up document to the Rural Health Unit for signature of the Attendant at Birth
  3. Return to the LCRO for processing of the registration and release of the registered document

Registration of Certificate of Marriage (Timely registered)

In ordinary marriage, the time for submission of the Certificate of Marriage for registration is within fifteen (15) days following the solemnization of marriage, while in marriage exempt from license requirement, the prescribed period is thirty (30) days, at the place where the marriage was solemnized.

Registration of Death (Timely Registration)

Registration of death shall be made at the Local Civil Registry Office of the Municipality where it occurred within thirty (30) days from the time of death, registration beyond 30 days from occurrence shall be considered delayed registration.

  • Duly filled-up Certificate of Death
  • Medical Record

Steps

  1. Sign in the client log book in the office
  2. Bring the filled up document to the embalmer for signature and at the Rural Health Unit for signature of the Attendant at Death and for review by the Municipal Health Officer
  3. Pay the required fees at the Municipal Treasury Office
  4. Return to the LCRO for processing of the registration and release of the registered document

Registration of Birth (Delayed Registration)

The birth of a child shall be registered within 30 days from the time of birth at the Local Civil Registry office of the municipality where the birth occurred, beyond this period will be for delayed registration.

Requirements

  • Negative result (PSA Census Serbilis outlet)
  • Baptismal certificate
  • Earliest school record
  • Government issued I.D.’s
  • Joint Affidavit of Discrepancy
  • Medical record
  • Other supporting document as required by the MCR

Steps

  1. Sign in the client log book in the office
  2. Submit the required documents to the MCR for initial assessment verification
  3. Bring the filled up document to the Rural Health Unit for signature of the Attendant at birth
  4. Submit to LCRO the duly filled up and signed COLB
  5. Pay the required fees at the Municipal Treasury Office and submit to the MCR the proof of payment
  6. Return to the LCRO after 10 days for release of the registered document

Registration of Birth of an Illegitimate child

Requirements

  • Duly filled-up Certificate of Live Birth
  • Affidavit of Admission of Paternity
  • Affidavit to Use the Surname of Father
  • Sworn attestation by the mother

Steps

  1. Sign in the client log book in the office
  2. Submit the required documents to the MCR for verification and assessment
  3. Bring the duly filled up birth certificate at the Rural Health Unit for signature of the Attendant at Birth
  4. Pay the required fees at the Municipal Treasury Office
    • Registration of Legal Instrument

  5. Return to the LCRO for processing of the registration and release of the registered document

Petition for Correction of clerical or typographical error in any entry in civil registry documents except nationality and status of a person

A clerical or typographical error refers to an obvious mistake committed in clerical work, either in writing, copying, transcribing or typing an entry in the civil register that is harmless or innocuous, such as a misspelled name or misspelled place of birth and the like, and can be corrected or changed only by reference to other existing record/s.

  • Certified machine copy of the certificate containing the alleged erroneous entry/ies

  • Civil registry record of ascendants/descendants

Steps

  4. Return to the LCRO for the processing & affixing signature in the petition
  5. Return to the LCRO on the date specified by the MCR

Petition for Correction of clerical or typographical error in the DAY and MONTH in the date of birth or SEX of a person appearing in the birth certificate

The verified petition for correction of sex shall be filed in person with the MCR of the Municipality where the record containing the entry in the birth certificate to be corrected is registered.

Registration of Legal Instrument – LEGITIMATION

Legitimation is a remedy by means of which those who in fact were not born in wedlock and should therefore, be considered illegitimate, are, by fiction, considered legitimate, it being supposed that they were born when their parents were already validly married

  • Birth Certificate of the illegitimate child
  • CENOMAR of parents
  • Marriage certificate-parents

Steps

  2. Submit the required documents to the MCR for initial assessment and verification
  3. Pay the filing fee at the MTO
  4. Return to the LCRO for the processing of the documents
  5. Forward the legitimation documents to PSA-OCRG, CRS Division thru forwarding company or post office

Application/issuance for/of Marriage License

All applicants (at least one of the applicant must be a residence of this municipality) of legal age intending to get married must apply for a marriage license at the Local Civil Registry Office (LCRO). The issued marriage license is valid in any party of the Philippines for a period of one hundred twenty (120) days from date of issue.

  • Barangay Captain certification that applicants planted at least 2 fruit bearing trees
  • Death certificate of spouse, if widowed

  4. Return to the LCRO and present the proof of payment and sign the application form
  5. Receive the Application form with the supporting documents
  6. Return to the LCRO on the date specified by the MCR

Supplemental Report for Birth, Death and Marriage Certificate

Supplemental Report using the appropriate form maybe filed to supply information inadvertently omitted when the document was registered

Steps

Issuance of Birth, Marriage and Death Certification

Certified transcription of birth, marriage and death registered at the Local Civil Registry Office this municipality shall be issued.
